Patellofemoral pain syndrome


 


Summary

Dx and Mx a challenge. Overuse and muscular dysfunction common contributors. Initial management includes quads strengthening. Check footwear and add arch supports if necessary.


Background

Retropatellar / peripatellar pain from changes in patellofemoral joint. Different to chondromalacia (frayed cartilage)


Causes

Pes Planus (Pronation) - "Flat feet"

Pes Cavus (High arched foot, supination)

Muscular causes


Key Clinical Features

Anterior knee pain with exercise (+/- bilateral). Often worse going down hill. Can also occur with prolonged sitting (movie-goer's sign)

Investigations

Consider plain x-rays if concern about Osteochondritis dessicans (image right - hover over) or rare neoplasm (red flags or lock / letting down)

Management


Prognosis

Most patients do well with conservative management.
