Community Rehabilitation and Support Team (CR&ST) - Criteria for referral

And at least one of the following

  • Previous falls
  • Difficulty getting up after fall
  • Long lie on ground
  • Unsteady gait
  • Requires mobility aid
  • Falls resulting in fracture
  • Community dwelling patient living within 8 km of city centre
  • Age > 64yrs
  • Presenting with a fall related injury
  • Cognition normal (MTS > 8/10)
  • No major communication difficulties
  • No active alcohol abuse
  • Does not require further in-patient treatment
  • Requires rehabilitation from two or more disciplines
  • Is self sufficient or has family support at weekends
